On-line appointment booking

You will need to register with the Surgery for “on-line” services to be able to use this facility. If you have already done so, please use the separate link form this site.

Appointments can be made online using the link here

Appointments can always be booked in advance, although you may have a slight delay if you wish to be seen by a specific clinician.

When you telephone the Surgery for an "URGENT" appointment, Reception staff will now ask "what is the nature of your appointment" or will ask you to "give a brief idea of the problem".  They will no longer ask you if the appointment is urgent.

Miriam, Sue and Casey have all been trained to be "Care Navigators" and are able to book you with the most appropriate Clinican.  This may not always be the Doctor, this may be Heather our Nurse Practitioner who is able to prescribe for you or Emma our Practice Nurse.  You can also be booked for an appointment with Mercy, our Clinical Pharmacist who will either see you face to face or who can deal with your query over the telephone.  We can offer appointments with our advanced Nurse Practitioner, Helen on a Thursday morning ho can see patients with complex conditions and prescribe.  We now have 3 MSK Physiotherapists that can be booked for NEW joint problems.  The Physiotherapists will be able to administer joint injections, request X-rays and other clinical tests and offer advice and exercises.

"Same Day" appointments will be reserved for seriously ill patients only.

It is unfortunate that we regularly have patients who do not attend for an appointment – this action wastes valuable NHS time (and money) so we ask you to ensure that you attend your appointments or advise us in advance if you are unable to do so.

Remember, it could be you one day who needs an appointment but cannot get one, only to find that others have not attended!

We would like to send you an SMS message to remind you of booked appointments (usually 24 hours in advance). This service does NOT mean that you should not remember appointment dates, and is merely a reminder. If you are happy for us to do this can you please arrange for us to record your mobile number – please remember to advise us if this number changes in the future!

Our patients are now able to access appointments at the weekend at 1 of 2 locations - currently at GPHA Clinic, Hadleigh, and Audley Mills Surgery, Rayleigh. You can book these appointments in advance by contacting our Reception team, or telephone 01268 206408 or 01268 740501 respectively on the day after 8.30 am - It is not possible for patients to attend without booking in advance.
